| Eligibility - Population(s) Served: | People with a diagnosis of stroke or other neurological condition, excluding acquired brain injury and spinal cord injury * needs cannot be met with community services * must have potential for functional and/or neurological improvement * must be medically stable and have sufficient endurance to participate in various therapies * must require services of a professional interdisciplinary team * must have achievable rehab goals * must be willing to participate in the program * discharge plan must be in place prior to admission | ||
